NOTICE REGARDING DISCONTINUANCE IN MAILING PROOF OF CLAIM FORMS THROUGH THE BANKRUPTCY NOTICING CENTER ON CERTAIN NOTICES

The Federal Judiciary has asked courts to help reduce the Bankruptcy Noticing Center’s program costs by taking steps to eliminate the printing and mailing of the claim form when sending certain notices. This request is based on a study that shows a vast majority of claims filed are created electronically by creditor’s software or through some other electronic forms creation tool.

PLEASE TAKE NOTICE that effective October 1, 2011 proof of claim forms will no longer be included with the following notices by our court:

B9C Chapter 7 Individual or Joint Debtor, Asset Case B9D Chapter 7 Corporation/Partnership, Asset Case B9E Chapter 11 Individual or Joint Debtor Case

B9F Chapter 11 Corporation/Partnership Asset Case

B9G Chapter 12 Individual or Joint Debtor Family Farmer B9H Chapter 12 Corporation/Partnership Family Farmer B9I Chapter 13 Case

A Proof of Claim form can be obtained at the United States Courts Web site at: http://www.uscourts.gov/FormsAndFees/Forms/BankruptcyForms.aspx) or at any bankruptcy clerk’s office.
